Port not forwarded correctly

could anybody help me to port correctly?

i tried the step by step guide of Portforward...but still cant get it to work properly

ill give as much info as i can so pls help me...

isp - Smart Wifi (philippines)

os - Windows Xp (sp2)

router - WRT54G V5 (firmware 1.00.6)

firewall - only uses windows firewall (has exemptions for the client)
